Instructions:

We will need all forms filled out and completed in order to get you started on your next project with Freedom Construction Corp. The W9 form link will bring you to a web fillable form that can be downloaded and/or printed. The subcontractor packet will need to be printed and filled out by hand. Then it can be scanned in and sent back to us, via email, as a pdf document. In the subcontractor packet you will be instructed to provide your "Certificate of Insurance (COI)." Freedom Construction Corp., along with our address, MUST be listed as an additionally insured on your Certificate.
